  • I would make sure that the medical practice has a strong IT support team.
  • You must have a trainer that can stay on top of changes and train new personnel how to use the software as it is complex and offers a lot of features.
  • A critical question is: Are you ready for digital records stored in the cloud and do you have the proper equipment for moving to cloud storage, i.e. Internet bandwidth, redundancy of service, reliable equipment, business continuity planning in place, and adequate support? A power outage, for example, can quickly turn a normal situation into chaos.
  • Is browser based software responsive enough for your needs? Often the move from client-server to cloud based is not adequately understood and technical needs are underestimated.
  • This solution is excellent for the smaller practice with a limited budget and limited time to focus on the technical details.

Pros

  • AdvancedMD stores all information securely in their cloud, eliminating the need for local servers, security monitoring, related security costs.
  • AdvancedMD stores all medical records digitally eliminating the need for extensive paper-based record keeping and the corresponding security concerns.
  • AdvancedMD integrates nicely with faxing services, scanning via Fujitsu scanners, and related equipment.
  • AdvancedMD allows for customized input templates giving the medical practice some degree of flexibility as their needs dictate.

Cons

  • Just recently we had an incident in which none of our employees could access the training.advancemd.com training materials. After extensive trouble shooting it was determined the issue was not with the local network, PC configuration, or firewalling. The AdvanceMD support personnel were not empowered to escalate this case as it should have been. Ultimately a correction in the server configuration for the training materials cleared up the issue, but not with assistance from Tier 1 support personnel.
  • Integration with the scanners can be problematic and has been a headache. Even with the Fujitsu scanners, the best in the business, we had challenges and found that some of the Tier 1 support staff were not trained or did not have access to the necessary documentation to fix the issue. The issues were eventually resolved but not without time lost in the process.

  • The look of the software needs and update.
  • When patients are web enabled they are sent an email of their appointment time to tell them in the email to arrive 15 minutes early rather then giving them the exact time of their appointment. This throws our schedule off.
  • The ability to enable to text a patient's phone regarding to remind them of their appointment date and time.

I think it can be intimidating at first but once you begin navigating and learning the modules, it becomes less difficult to perform charting and operational processes.

It's very important to limit your schedule during the weeks after go live but it is equally important to have a resource that is the lead at the practice that ensures that milestones are met leading up to the go-live date. Someone must be the point person at the practice otherwise milestones will be missed and the implementation will run into problems.

Alternatives Considered

ADP AMD is much easier to learn and navigate than most all of the systems I have worked with, including Greenway and Centricity. The only thing I liked better in Centricity was the ability to run more customized reports, but overall, I would choose AMD any day over those two.

I've also used NextGen and Epic. I believe that Epic is by far the best EHR on the market for medium-large healthcare organizations. There are downsides however including the cost of the Epic solution which makes it cost prohibitive for some organizations to implement Epic. NextGen is more geared towards specialties, in my opinion, and is in a similar market as eCW. With that being said, I believe eCW is a much better solution than NextGen based upon ease of use, price, functionality and support

  • I will just share one area that our organization saw the ROI in a very short time period. That is the elimination of a dictation service for most of our specialty group doctors when we introducec Dragon Medical. This functionality brought a tangible benefit and a significant ROI in a short time period.
