Applied Epic

Agency management for larger P&C driven agencies that are looking for standardization across their business. The system really allows you to track and see when activities are being created and completed, where an account is in the marketing process (or renewal process). It also is a great data warehouse when looking to consolidate.

For a clinic setting I would rate the system much higher as that is the application that the program was designed to work for. The flow of the system works for a clinic but is cumbersome to use in a surgery center setting. We agreed to use this system because the clinics in our company used the system. It was helpful to have the ability to pull information from the clinic setting to the surgery center. When our clinics closed and only the surgery center continued to use the system, that value was gone.

Pros

  • Account Management - I like the way the system organizes the data for each account. It is easy to search and find any client we are looking for and then once within that record, it is easy to navigate to the necessary policy or screen.
  • Task Management - once everyone was trained and on the same page, the ability to move tasks within the system from one person to the next really helps with the "passing the baton" from one team member to another. The task will come right up on the initial screen so the user knows they have something new to do.
  • Reporting - the system drives the majority of the reporting we do as an agency and it is great when working within the system.

  • Athena is able to create a medication list from the patient's pharmacy purchase history. This aides in compiling a comprehensive list quickly, especially when the patients do not know what meds they are on. The downside to this module is that every time that you add a medication that is not on the list, it turns it into an order that the provider needs to sign. There are many times that you need to make an addition, that the provider does not want to sign as an order.
  • The patient history module is easy to configure to meet the needs of the organization.

Cons

  • Employee Benefits Agency Management - we found that Epic could not be used to support our EB business and as such now have a bridge between the two systems. It would be fantastic if that was all integrated into one.
  • Contract Management - its hard to management/update account specific contacts especially for large companies that have changed and many points of contact

  • There are many areas where Athena can improve. The support for surgery centers is non-existent. I was told that 2017 was going to be the year of the surgery center with opportunities to improve the system for that application. This never happened and the team that was supposed to assist me was disbanded.
  • The list of prior surgeries is cumbersome and difficult to use unless you happen to know the CPT code for the procedure. We end up choosing the closest procedure that we can find and making a note to clarify what we are listing.
  • The medication administration module is hard coded and cannot be changed. There are options that are absolutely ridiculous but we are told that they must stay that way. For example, an IV fluid rate is set at ml per minute rather than the standard of ml per hour. IV start sites are anything from the hand to rectal. When you give a medication it assumes that you are doing so based on a verbal order and requires that you document the verbal order. We give meds based on written orders almost all the time.

Overall the system is good. Actually, this was supposed to be the universal EMR when it came out during the Bush administration.

Alternatives Considered

Applied Epic was more robust than some of the other systems we looked at. It had the ability to be configured to work with very large fortune 1000 companies all the way down to personal lines business which was advantageous for us.

I was not the one that selected the program but we all voted as a group and the CEO made the decision. We actually voted to do more research because of the expense of the program. It is easy to use and to learn with many available templates. We are in psychiatry and there were not initially a great deal of templates but there are now. All in all it is a good program but expensive.

  • Consolidated from multiple systems to one for P&C - reduced spend on maintenance of those systems and centralized our support making life easier for IT.
  • Positive impact on the time spent actually putting information into the system. Previously TAM was used and that system was hard to navigate and use.

  • The initial cost quoted was accepted by our CEO who did not spend a great deal of time negotiating. It was very high.
  • The cost became prohibitive after a few months and we tried to renegotiate rates. There was a minimal discount given.
  • In our local area, most of the practices that were using Athena changed due to cost issues.
  • I started a new practice and called to discuss continuing AthenaClinicals. They started with a very high price and I was able to negotiate down almost 6 percent. Even with that discount, it was significantly higher than its competitors.
