We have used Patient Point. It's difficult to use and unreliable. Atmosphere has blown it away.

Interact is great for basic integrated quizzes and forms. The other tools are much more robust in terms of mobile friendliness, integrated payment steps, file upload, Google Tag Manager integration (track and segment via your favorite marketing automation platform), etc. Typeform is a much more robust platform.
