I use Bonterra Case Management in my organization by obtaining resources to meet my clients needs whether that be in a nearby city or town or in a different state. The business problems that Bonterra addresses involve issues around networking, funding, and resource guides. The scope of my cases involve medical care, mental health care, stable housing, food and nutrition resources, transportation, and family/social advocacy.

Program Delivery

In the process of achieving this benefit. Bonterra Case Management has allowed the ABCCM to distinguish between who is obtaining the resources they need and who is being stability housed once their stay in our GPD program has been completed. Bonterra has allowed our case managers to have a greater degree in influence in understanding of our clients and their needs.

Diversified Funding

Bonterra has helped the ABCCM to navigate populations within the community which are still struggling to make ends meet. Bonterra allows the ABCCM to have quantifiable statistical values of disenfranchised populations and the resources which still need to be delivered.
