The majority of our usage is digitizing our forms, especially without clinical teams. We have Clinical assessment forms like the Braden Scale, Abbey Pain Scale Management, Delirium Screening, FRAT Assessment, Oral Health Assessment, Cornell Scale Assessment, Resident Risk Assessment, Bed Rail Assessment, and so much more. Other usage is for our staff to update their own records without having to log in to Salesforce, which gives them a much convenient way to update their own records.

- Data Accuracy - Form assembly has validation rules that help with fewer errors and lead to more accurate data vs handwritten paper forms.
Form Assembly allows for advanced data mapping, pre-filling forms with Salesforce data, and working with custom objects. FormAssembly excels in Salesforce Integration, as it is highly specialized and deeply integrated. Often considered the industry leader for Salesforce forms, Formstack has strong integration, but may not offer the same level of granular control as FormAssembly.
