Formstack is being used across our entire university. It is our enterprise form creation and workflow tool used by various departments across the campus to create forms and workflow.

Easy & intuitive form builder that allows users to create forms

Logic within forms to show and hide fields
Logic with email triggers to only send notifications when certain answers are filled out in the form

Their workflow feature is still half-baked. Lacks enhanced features like logic & ending the workflow mid-step
Some of their integrations are outdated and no longer supported so basically useless to our organization: ie.: Zendesk Integration
No integration with Active Directory so user management is a pain

It has great features and they continue to add features. Formstack works great for standard forms but has many limitations with workflow forms, the biggest limitation being you can only route forms to Formstack users.
It is being used on a limited basis to automate workflows for tasks like audit requests, system access removals, etc.

There are a few situations that we have encountered that required more complicated workflows that were not very intuitive to set up. We instead opted to use Formstack for submission, then moved the workflow to a dedicated tool.

We use forms from Formstack to implement applications for pilot testing, as well as for certain lead retrieval tasks that Hubspot is unsuitable for. We also use it as a tool internally to manage group events and to collect information about the team, as well as a type of internal ticketing system for various requests (design, copyediting, etc.)

Managing form designs in Formstack is very difficult, and we've had numerous problems with design/styling.
Certain features are missing from the form builder, like "same as above" functionality.
The form storage backend is difficult to navigate, and it is difficult to find forms in any way but searching for them.
Sometimes features in the form builder require reverting to the old form builder to implement.

For well-funded institutions with a large number of forms needs, Formstack is well-suited for a variety of tasks. However, the styling limitations held us back numerous times and the we've reached the form limit frequently and have had to upgrade our account. For smaller, less well-funded organizations, the cost would quickly get prohibitive if they use it like we do.
Formstack is managed by several people within the university's public relations department, but the forms are used for everything from registrar, event registration, admissions, service requests and many others. Using Formstack allows us to have a centralized place where we can create and host easy-to-build and easy-to-use forms that provide simple and reliable assessment for the back-end user.

Formstack is good for basic event registration and information gathering, particularly if the information will be collected on a regular basis.
Formstack is used by various departments in our organization as workflow, setup form. We use it to on board new customers, track fuel use in our delivery trucks and submit requests for new crew members. It gives us a place to track requests and make sure the correct fields are filled out.
Pros
The triggers in Formstack to hide / unhide fields based on other answers is extremely helpful. It keeps the form on target and doesn't clutter up the question list with useless fields.
The forms are extremely easy to set up and deploy.
We don't have to worry about supporting the backend or writing the scripts for the work flow.
Cons
The only issue I had with when starting with formstack was the workflow/approval. This has since been corrected.
It would be nice to have the data file that is created from the form submission to be formatted in a certain way so it can flow into an automated system.
Options for data file delivery would be nice too, FTP, SFTP or email.

It's very handy for onboarding new customers, crew members and product setup.
We use it extensively within our department for various applications, surveys etc. The customizable features and website embedding work well with what we are trying to accomplish for our data collection purposes. Departments and schools within NYU make purchasing decisions separately, so we are not aware of other departements usage.

Customer service was way more complex than necessary when we were trying to merge two accounts (unknowingly) created within our department.
The website embed has occasionally given us some trouble as well.
Users have had various issues with radio buttons and file uploads over the past two years I have used Formstack.

Overall, it has worked well for us. Cost is prohibitive for smaller teams. Templates are not simple, and it is easy to break especially when there are multiple team members using the account.

Only from personal experience can I make this statement. In our department we use Banner (university wide) Formstack and KX and none of them "talk" to each other. If it were easier to link them together, or cheaper as cost is always a factor, it would make our job significantly easier.

We use a custom form to have attendees log in to their event via a link we email the organizer. They are able to provide all their info with a picture making it easier to track the number and the photos for the event and create the ids for each.
