Overall Satisfaction with Jotform
The forms are VERY easy to create and even easier to use. There are pre-defined forms that you can leverage as well. The other great thing about this company is their customer service- a few times I went on the chat as I couldn't figure out how to do something and they were able to help me. There are also a lot of informational videos online to help with form development.

There are more options with Jotform than with Google Forms but Google Forms is better integrated with live xls and form outputs.
