We use FormAssembly for grant applications, inquiry forms, and other external client forms. These forms are all connected to Salesforce, using the FormAssembly integration to our organizations Salesforce.

Pros

Easy to navigate for creating forms
The new AI features for form building

Cons

I wish there was a way for the submittor to receive a PDF attachment of their responses
Logic is difficult for sending notification emails. I wish there was more support there

FA serves as the main platform for global students and staff to submit requests, with information sent to connected systems, primarily Salesforce. It is used by all current Oxford University students, not just those at business school. Teams such as the CRM Team, Web Team, Entrepreneurship Centre, Goldman Sachs Team, and Programme Recruitment and BD teams use/ manage FA for various purposes. The system is user-friendly, easy to configure, supports branding, and incorporates custom logic and rules, making it very helpful.

Busy Support channel and no conference call support.
Weak alerting system make failure/incident management hard.

Use Cases and Deployment Scope

We use FormAssembly to register adult learners for professional development trainings, and to collect evaluations at the conclusion of the sessions. We are able to pass the information directly to Salesforce and track the responses easily through Salesforce and email alerts. FormAssembly also allows us to screen people for courses that have prerequisites, and collect different relevant data based on responses.

Easy to add branding/logos to forms
Conditional questions for screening and follow up
Being able to save predefined field sets and clone existing forms is super helpful!
Ability to connect to Salesforce, Google, etc is really useful

Cons

I really wish the form list was wider! A lot of our forms have similar names and max width of only 800px with no text wrapping makes it difficult to find when searching!

FormAssembly is really great for registrations, applications and collecting feedback. There are some times when we don't use it for feedback because it doesn't create graphs, and apparently the system our coworkers' department does. This is only an occasional issue for our department, so I don't really consider it a problem.

Use Cases and Deployment Scope

We use FormAssembly to build forms to enable external stakeholders (including customers and contractors) to submit data into our Salesforce instance. We use Salesforce across many aspects of our operation, but Salesforce provides fairly limited options to create forms allowing non-Salesforce users to send data into Salesforce. We've found FormAssembly's easy-to-use Salesforce connector to be invaluable in plugging the gap and allow us more options to intake data.

Pros

Salesforce prefill is a powerful way to prefill forms with records from Salesforce and prevent duplicate data entry.
Salesforce submission connector allows us to create some pretty complex and powerful logic to process form submissions into Salesforce data.
General form prefill options via query strings in the URL is a helpful feature.
The ability to use custom CSS and javascript on the page makes the forms very extensible and has been helpful to give us even more functionality.

Cons

Improved design and templates could make it faster for us to create mobile-optimized, beautiful forms - we find the current templates are functional but we have considerable work to do in cases to make them feel more in line with our branding.
We would welcome additional security and authentication features on form submission, including potentially 2-factor authentication in order to verify that a submitter matches a user in our database, or IP restriction on who can submit.
We would love to be able to organize forms across more levels of hierarchical organization, such as using folders/directories to organize different types of forms.

Likelihood to Recommend

Our primary use case for FormAssembly involves using it for data intake into Salesforce. We've found that FormAssembly for the cost is the best and most flexible form builder on the market to allow us to quickly create and test forms to get data into Salesforce. We've even been able to accommodate quite complex logic in FormAssembly forms to process form data into actionable information internally. Outside of the specific Salesforce power-ups, I probably would not be as keen to recommend FormAssembly over other applications such as Google Forms. While we love the Salesforce connectivity, I would say there are other apps on the market that offer better options in terms of sharing settings, user validation, and overall look and feel of the forms.

Workflows are extremely helpful to handle reimbursement requests from contractors that then get funneled to the appropriate department for approval. It even sends the e status notification. <div>

Cons

Reporting filters and list views on workflow responses aren’t always intuitive. It gets the job done though.

We use FormAssembly to support the development of online forms as part of our programs that support tourism promotion for British Columbia. We needed our forms to be easy to build and to feed data into our Salesforce instance. The use cases are primarily around registration of users into our systems to join the tourism-related programs.

Pros

The interface is easy to understand
The integration with Salesforce is seemless
It retains the form data in case we need to access it later as a backup

Cons

we could always use more styling options when it comes to the form builder
in our past use of the form builder when adding attachments to a form, there was an upload limit of 35MB. It would be great if this limit was increased.

Likelihood to Recommend

FormAssembly is great if you need to build forms quickly and efficiently with a simple workflow and one way data flow. For scenarios where you require a logged in experience for a user to save and come back to their form submission we found it less suitable.

We use FormAssembly to populate client information to populate into an agreement we've created. All information is assigned to a data point within the agreement. This information is then transferred directly into the agreement, which populates the agreement. FormAssembly is essential to gathering all client data in a clean and concise format.

FormAssembly is a solid option to capture data input to be transferred in to an agreement already written. However, it requires the creation of the data points, which can be cumbersome. I could see FormAssembly not being as much of a use to agreements that are not standard and will need a lot of customized fields.

Use Cases and Deployment Scope

Our main use of FormAssembly is for our lead form, which we put on our website landing pages and share with our lead vendor. This allows us to have one main front door into our Salesforce org. We also have an internal lead form that our staff use to fill out when speaking to leads on the phone. There are a few other forms and workflows in our organization that I'm excited to eventually move into FormAssembly. This will make the processes easier for both our staff and customers, and improve data integrity as we can connect the forms to Salesforce.

Pros

Easy basic form-building
Easy integration (Salesforce)
Helpful knowledge base

Cons

The option for an occasional call and screenshare would be nice.
Administration and setup (user management, granting access, etc.) could be simpler
While a basic form is easy to build, I wish customization (text, size, placement, etc.) were easier without knowing CSS.
It would be helpful to be able to subscribe to reports - that is, instead of manually downloading a CSV each week, receiving an email with the list of responses specified.

Likelihood to Recommend

FormAssembly has worked very well for our organization and has met our needs so far. It seems to be one of the top choices for organizations that use Salesforce as their CRM. I would give it a 9 and not a 10 only because I don't know the competition and because of the areas of improvement that I listed previously.

Use Cases and Deployment Scope

The majority of our usage is digitizing our forms, especially without clinical teams. We have Clinical assessment forms like the Braden Scale, Abbey Pain Scale Management, Delirium Screening, FRAT Assessment, Oral Health Assessment, Cornell Scale Assessment, Resident Risk Assessment, Bed Rail Assessment, and so much more. Other usage is for our staff to update their own records without having to log in to Salesforce, which gives them a much convenient way to update their own records.

Easy Connectors understanding.

Cons

Easier Page customization.
More Aesthetic templates.
CSS editing isn't for all users.

Likelihood to Recommend

Digitizing your company's physical paper transactions. Once digitized, your staff/users can easily access it with their mobiles or anything that has a browser. This gives them convenience and saves paper waste. Some scenarios I think are when some people would like to treat from assembly as a document signing platform like DocuSign or Conva.

It's great at letting our program participants enter their own data that then flows conditionally to our Salesforce org. We validate data on the front end, in FormAssembly, before Salesforce even sees it.

Cons

Integrations with other databases (we use Sage Intacct for finance)
More centralized/blanket meta-management of forms
Webinars are often skimpy on instructional content and frequently feel more like commercials
Would be great to establish Salesforce Connector Groups or references that can be applied to multiple forms at once -- it's a pain to reestablish user credentials on each and every single credential when our goal is to use a single service account on all of them.

Likelihood to Recommend

FormAssembly is great for front-facing form entries where sophisticated back-end logic might be useful, or where data transformation could help with hygiene in a destination database. We love the Salesforce Prefill Connector that ensures our form fillers are always using the most current available picklist options and lookups when adding data to our database.

Unfortunately, we have found that the relative absence of other kinds of connectors means we have to pass FormAssembly data into our Salesforce org and then pass it onward from there, which requires additional administrative overhead to manage cleanly.
