Overall Satisfaction with Google Forms
Google Forms was used to start up assisting with my hospital's "Youth 0 Suicide" initiative. It was used in the psychiatric department as a way to start analyzing the trends in people that came into the program. It was initially used to get a visualization of what we were working [on] then, and eventually transitioned to Access as the data became more apparent. Now, we directly translate the data we receive, and then put it into our own database. It helped create a basis for what we were doing.

Google Forms sometimes does better in terms of accessibility, however SurveyMonkey gave more in depth reporting functionality, with easier accessibility to customize questions for surveyors.
