I highly recommend Jotform
Overall Satisfaction with Jotform
At the start of my therapy career, I tried several ways of collecting client intake data including fillable pdf forms, Google Forms, word documents etc. None of them gave me what I needed. Then a friend sent me a Jotform form. I cloned the form and have been using Jotform ever since. Jotform gives me all the functionality I need and then some and the ability to electronically sign. It is so easy to use and meets my needs perfectly, enabling me to quickly craft any form that I need. Also, when I am ready to scale, I know that Jotform will be able to scale with my business.
Pros
- I love that I can get a clients SUDS score for multiple symptoms in a single 2 dimensional grid in which the clients can simply click buttons instead of having to write.
- I also love the flexibility of the clone feature. It makes it so easy to create variations of a design.
- I love the electronic signature feature. This allows me to keep everything in one place instead of using multiple discrete documents.
- Supremely easy to use and I have already recommended it to several non-techie therapy colleagues and they love it too.
Cons
- Honestly, it meets all my needs so I have little to say here. If you're talking bells and whistles, maybe a degree of animation might be in making the form more eye-catching.
- Thinking about it, maybe I don't know how to use it properly but it would be nice to link one form to another for "filing" purposes. For example I have a client, after the first session, I have a bunch of data. I would like to send a form to the client after a week and the be able to attach that reply and all subsequent updates to the original submission.
- I don't have numbers but it has saved me time and effort when gathering client information
- The time and effort to create a new form is minimal. For example, for a new type of client, I cloned an existing form, modified the clone, and published it within an hour!
Jotform beats them all in terms of ease of use, flexibility of designs and ease of use
