athenaIDX is an on-premise or cloud-hosted enterprise RCM solution that helps health systems, hospitals, billing services, and large practices optimize financial performance and successfully adapt to healthcare payment reform. It is based on the Centricity medical practice and EMR software that was formerly owned and supported by GE Healthcare acquired by Veritas Capital in 2018, spun off into a new entity (Virence Health), and then rebranded as a product of athenahealth (also a Veritas…N/A

I couldn't recommend IDX to anyone. Most of the reviews on this site appear to be for the program that broke off from Athena IDX, Centricity Practice Solutions (CPS). CPS is a much more capable program managed by Unlimited Systems (a separate company) that should NOT be confused with Athena IDX (managed by Athena Health). It's possible that Athena IDX was usable in the 90s compared to other peer programs at that time, but there are so, so many better, modern solutions available now for far less money. This software is priced at the enterprise level with a level of quality that is below budget tier with an ancient feature set to boot.

Cadence Enterprise Scheduling is particularly useful in a multiple provider, multiple location scenario. It is also quite useful in situations where there are multiple rooms or pieces of equipment that need to be scheduled.

Zuora
Zuora is great for smaller subscription businesses that have a limited number of simple or basic products. As more products are added to the catalog, the ability to report on specific product performance has become more challenging. Similarly, as our number of customers has grown significantly over the past few years, the extraction of data through export files is more of a challenge to capture all data points.

Pros
athenahealth
  • One of the best things about Centricity is it is flexible. We are able to add a data point behind a term (CPS calls this an observation term) and track any data needed. These observation terms also allow us to pull information forward and display information needed to make a good decision or used to decrease errors. For example in our history forms given to the patients one of the questions is side of injury. This affected side displays on our EMR forms which helps providers and prevents mistakes.
  • CPS integrates with other programs. We are paperless and use DocuSign to send each patient their history, ROS and all new paperwork. This allows us to import the history into the forms and has saved us from adding extra staff (FTEs) to room and take history. The history form can be completed at home or in the waiting room. Each section has a observation term behind it and allows it to be placed into the note at the correct section. Since we have a fully integrated product we do not have to have another program to enter or transfer our charges from the EMR to the PM side of the program.
  • Our notes look great! We did a lot of customization which gave us the detail and look we wanted. It seems like a lot of work at first, but the outcome has so much efficacy. We increased our providers without having to increase our staffing and it is due to he efficiencies we gained by using the EMR.
  • One other note, Centricity is flexible but that means it is complicated. You can not have flexibility without it be complicated don't let that stop you the end results can be great.

  • The system is simple to use it helps schedulers create appointments and, avoids placing patients in situations where they are scheduled at the same time in different facilities.
  • It has many reports that can help managers maintain a smooth operation and shows what areas need to improve.
  • It has a one place for all scheduling information for schedulers to see patient's appointments, especially when patients do not recall where or when their next appointments will be.
  • The system can be created in such a way that you can control who can make appointments and who can't.
  • They system when built provides warnings to schedulers helping to minimize many scheduling errors like overbooking.

Zuora
  • The implementation of Zuora is very helpful. Their interactive training guides are one of the best I've used. It is very thorough and any salesperson can learn how to use it very quickly.
  • Their subscription management offers many ways to sell a subscription. These subscriptions can be evergreen (renewing every month) or termed subscriptions. The subscriptions can be amended to add/remove products very easily.
  • The ability to quickly pass through large amounts of usage data for our clients is allowing us to perform billing in a time-effective manner. Once usage data is loaded, a few clicks and all of the invoices for our client base can be generated. Invoices are clean and detailed which help us maintain a good customer experience even with our somewhat complex billing model. The reporting is extremely helpful in calculating the end of month commissions, recognizing and deferring revenue, and overall bookkeeping.

Cons

  • Statements print only current open accounts, would be nice to print a total statement for patients so they can see if payments are split between open and closed/zeroed out accounts
  • Ability to create custom reports

  • Questionnaires: EPIC seems to be very stringent on questionnaires and how may can be attached (only 1) on a Cadence level causing us to be quite inventive on how to get questioned answered on only certain visit types. One big stumbling block was the Ebola questionnaire. Currently we have a particular questionnaire for new Phy. Therapy patients which prevents us from adding the Ebola questionnaire to NEW patients. We are still struggling with this.
  • Patient Itinerates used for multiple appointments: Not very professional looking regardless of how you set up (OP visits, instructions etc: Most practices will not use the itinerary therefore adding dollars to having printing cards with the practice name on them. (the old fashioned way)
  • Building new DEP records: Wish there was a way to copy entire builds from one DEP to another.
  • Security": EPIC security is a nightmare to develop for multi faced users. RN that schedule appointments. Many of our practices are small practices that only have 2-3 people (the doc, the RN and the scheduler/biller . This creates the need for everyone to do everything from a scheduling perspective and building this security can become very cumbersome from practice to practice as each is unique.

  • User Interface - Zuora's UI is still a weak point, however, this is negated through the use of its API's. Zuora has done a lot of work in releasing new products to limit its UI experience and the addition of Orders allows for more functionality to reduce the UI issue.
  • Complex system - This is both a pro and a con, but in this instance, it is a con because we may not have taken the time to truly understand the implications of all the options that were available. the Flexibility is great however if you do not take the time to understand the product and what problem you are trying to solve, you can make life difficult for yourself later down the track. Take the time to map out your approach end to end and ensure your assumptions at the start are robust.
  • Reporting - reporting is weak as such we have moved away to our own reporting data warehouse.

Our business is now, more than ever, focused on our core business rather than homegrown support tools for quoting, contracts, billing, invoicing, payments and the rest of the subscription economy. We evaluated other solutions and found this the best and most viable solution given the strong ties to Salesforce and it's integration. Zuora works, and it works well.

As stated earlier, it seems to be the best out there, from a built perspective as well as a user application. While some practices feel it to be time consuming (Cadence/Prelude) together others who see the whole integration thru the Epicare and Resolute systems love it. From an IT perspective, while sometimes cumbersome to build, the end results are pleasing and satisfying. We can usually accomplish what we set out to do with the practice vision in mind.

I had 20+ years of accounting experience before taking on the revenue role. I had several things to [learn] but was able to easily master the software in a matter of weeks. Zuora is our preferred billing platform that we currently use - very efficient and much more automated than our other platform/process.

EPIC has been great in their support and communication at all hours. They also openly communicate when an application TS (Technical Support) will be away and who is covering for that person. A support request is emailed to their support team, then an automated response is sent to you in a couple hours saying "Hey, someone will take a look at your support request soon". Then somewhere around 12-18 hours later, an actual support member responds with "Have you checked out our tutorials? Here's one that sounds like it might help you". I don't want a tutorial given to me that I've already read through that only exists because of the terrible user design of Zuora's user interface

Use implementation team from EPIC but know what you want and make sure that it=s what you get. You can be easily swayed by lack of knowledge on the application and therefore decisions are made that are not correct and then have to be redone. EPIC including Cadence needs a great understanding of the workflows you currently have and how they will be impacted by your build. We had a great understanding of the existing workflows in the offices, however lack of knowledge on the Cadence IT side, had a large impact on what was delivered as our implementers believed that a standard EPIC workflow would suffice and did not.

Zuora provides way more functionality than Recurly or SaaSOptics as a billing engine. However, it is not a general ledger system like NetSuite ERP, it is still designed to be primarily a billing function that feeds into a general ledger via automated general entries. It has simplified our invoice management and automated billing while connecting with Salesforce for high-powered reporting and cross-departmental collaboration. The automation of revenue recognition has the potential to significantly reduce the time spent on accounting close by a few days.

Return on Investment
athenahealth
  • Extremely expensive software for the limited feature set. We're talking millions of dollars with off hours support (anything outside of 8-6EST) that starts at $1200 for the first hour and $300 for each hour afterwards.
  • Multitasking isn't a thing unless you assign more licenses (thereby increasing your costs). Users can quite literally only have one IDX window open at a time unless you assign more licenses. This slows down everyone as there are plenty of times where examining different parts of a patient's profile is useful.
  • Reporting has been nearly impossible as we use modern reporting software. IDX's analytics site is firmly stuck in the 1990s and lacks decades of innovation present in something like Power BI.
  • Nickel and diming philosophy. A limited API server costs ~$10k/month. Setting up a new facility in IDX costs you and requires a 4 month lead time. Many more instances of this.
